Preserve workspace-scoped project instructions across sessions - #82
Project instructions are part of the harness's correctness boundary. Previously, project-instructions loaded startup guidance once from process.cwd(), reused that global snapshot for every session, and lost nested discovery when session resources were disposed. A task agent in an isolated worktree could receive the launching checkout's rules and fail to discover its own nested guidance.
This was selected as the highest-impact bounded improvement after two independent repository audits and static inspection of the installed Claude Code 2.1.319 artifact. Claude Code was not launched or prompted. Its embedded instruction traversal/restoration code provided a comparison point; this is static evidence, not a claim of observed runtime behavior or measured task-success gains. The initial broad REA analysis hit a heap limit; focused, byte-ranged function analysis succeeded. Extracted proprietary source and research artifacts are not included in this PR.
Changes
Load startup and nested AGENTS.md guidance per session workspace, including worktree subagents, without changing the process working directory.
Add optional, plugin-independent asynchronous prompt preparation with typed, secret-redacted state in the existing session log. Persist changed state before provider execution; block the request on preparation/persistence failure.
Preserve discovered instruction paths across compaction, fork and resume, rereading current files on restoration rather than replaying tools or storing stale instruction text.
Clear discovery on /new and workspace changes; retain existing byte budgets and legacy session loading. Old logs without saved discovery rediscover nested instructions when files are used.
Document the lifecycle and prompt-preparation extension API.
bun checks:fix passes: native checks/benchmark, lint, formatting, typechecks, 885 CLI tests, script tests, builds and release check.
Initial independent $code-review found two edge cases: nested reads after a workspace-switch tool in the same batch, and cancellation during state persistence. Both were reproduced in failing regression tests and fixed; shared initialization preserves concurrent discoveries and an abort check prevents the next provider call.
Self and independent final $code-review: Standards PASS, Spec PASS; zero remaining blockers, all 15 files / 37 hunks covered (662 additions, 155 deletions). Both original findings were confirmed resolved.
Final-head CI (199e113cf513afe4b7a07d11c7c39d692eb6f889): main checks plus all eight macOS/Linux/Windows release builds passed.
CodeRabbit completed its initial review on 20553a6 with no actionable code issues. Its generic docstring-coverage warning is not applied because root AGENTS.md explicitly prohibits code comments. The follow-up on 199e113 was skipped (NEUTRAL) due to the hourly included-review limit, not treated as approval; self and independent subagent reviews cover the final fixes. No review threads remain.
Scope boundaries
This does not add a pre-mutation instruction gate or live file watching. Nested discovery still follows successful file tools, and a running session retains its loaded content until a lifecycle reload. Missing files are skipped; other loading errors remain visible. No new dependencies or cross-plugin dependencies.
The change adds asynchronous prompt preparation and session persistence for prompt state. The project-instructions plugin now loads and discovers instructions per session workspace, and restores discovered paths through session state.

Summary
Prompt preparation contract and registry apps/cli/src/agent/prompt/registry.ts, apps/cli/src/agent/prompt/registry.test.ts, docs/plugins.md
Prompt sections can define asynchronous preparation handlers. The registry checks handler IDs, cancellation, and returned JSON state, then saves valid results.
Prompt state session records apps/cli/src/sessions/types.ts, apps/cli/src/sessions/records.ts, apps/cli/src/sessions/records.test.ts, apps/cli/src/sessions/recorder.ts, apps/cli/src/sessions/store.ts, apps/cli/src/sessions/store.test.ts
Session records now store prompt state separately from conversation events. Loading a session collects that state and clears it after a workspace change.
Sessions prepare prompts before turns and compaction. They restore, redact, compare, and persist changed state, and clear it on reset or workspace change.
Workspace-scoped instruction loading and discovery apps/cli/src/plugins/project-instructions/*, apps/cli/src/agent/session/project-instructions.test.ts, docs/commands-and-skills.md
The plugin loads instructions for each session workspace and discovers nested files after successful file operations. Tests cover workspace isolation, state restoration, compaction, fork, resume, reset, and failure cases.

CodeRabbit completed its initial review (20553a6) with no actionable code issues. Its docstring-coverage warning conflicts with the repository's explicit no-code-comments rule and was not applied.
CodeRabbit's final-head follow-up was skipped/NEUTRAL because the hourly included-review allowance was exhausted. This is not a final-head bot approval; self-review and independent subagent review cover the final fixes. No unresolved review threads exist.
