chore: enable agent runs with structured results - #460

- Enable `agent_run` and extend it with `ultra` effort, budget limits, open provider IDs, validated run IDs, structured output schemas, and a 45-second default call window. - Add search warning logging and model-visible result notices, add `lite` search support, and reject unsupported advanced `lite` parameters before requesting the API. - Add `EXA_API_BASE_URL` client configuration and register `web_fetch_exa` through `registerTool` with title, input schema, and annotations. - Add sanitized search note types, billing guidance for agent credit errors, and non-error metadata for free-tier rate-limit notices. - Test agent inputs, progress, errors, structured tool registration, header encoding, rate-limit metadata, and search warning and notice sanitization.
