Summary

Triage now adds no labels unless the policy file sets labels: true or a map that renames them. A policy without labels:, or no policy file, means no verdict labels and no Triage: Failed to Run.

• A missing labels key reads as off; a repository's own key still wins over extends.
• With labels off, the first write Triage makes to a pull request takes its own old labels off, such as Triage: Ready. This uses the mirror's existing owned-label stripping, so no new code path was needed.
• Exporting a saved policy that uses the default label names writes labels: true, so the labels survive the round trip.
• The starter policy file and its preview carry no labels.
• Docs: the User Manual now says Triage adds no labels unless the policy file sets labels: true, and its label section and Failed to Run text are trimmed to match. The setup skill's reference is updated too.

Verification

• bun run check and bun run lint pass.
• bun --conditions=intx-src test packages/triage-contracts packages/triage-workflows packages/triage-manager packages/github-tool tests/model tests/policy-corpus: 544 pass.
• bun --conditions=intx-src test --parallel=2 tests/chain: 13 pass. The chain policies set labels: true.
• New policy-corpus fixtures cover no key (off), labels: true (on) and a rename map (on), checked against the policy the hub runs.
• New model test: a stack with labels off adds no label, and removes an existing "Triage: Ready" exactly once across two heads.
