Add twelve external plans from the 2026-09-02 program-intake triage (issues #199, #213, #230, #232, #234, #238, #239, #250, #252 plus three promoted residuals), each with typed dependencies, a landing-event impact table for the tool-pack-scope-provider-truthfulness merge and draft PR #190, drift checks, and revalidation triggers. Record them in a Wave 4 plan index, reverse-link the twelve backlog items, correct two item premises found in reconnaissance, and refresh the execution program: the docs-index exclusion plan joins W1 as a blocked successor, patch-and-restore joins W2 group 3, and the other ten compose a new W5. Disposition every finding of the 2026-09-05 Astra review (3 Critical, 19 Important, 9 Medium) and apply the accepted fixes across the 31-plan corpus, the execution program, and the W4/W5 indexes. Corpus-wide: lane instructions that still bumped the five packages or ran release gates now use lane/standalone verification modes; the version-pin seam in validation/skills.test.ts is recorded as a shared write with reciprocal rows; PR #190 landing rows corrected against its full 217-file list. Program: three verification modes, hidden-seam rule, drift-record rule, W4 regrouped (gate override + restamp, then dispatch stamp), W5 groups 3 and 4 made sequential, rationales corrected. Substantive: docs-index output never targets the scaffold's authored docs/index.md or mkdocs.yml; patch-and-restore fails closed with a binary-safe capture script; the retirement sweep runs before the log seal; gate-override, terminal-status, and quick-route own their oat-project-next consumers; recap preflight includes the set planner; active-pointer resume narrowed to post-archive receipt failure; readiness validator gains legacy mode and inspected-HEAD provenance; metadata-version drops its parser premise and the check:skill-bumps severity conflict. Declined: the W5 wave split (renumber churn; every group already ends in an integration checkpoint). Recorded as exception: the W2 repair split.
