fix(hook): rewrite producers before display pipelines

Summary

• rewrite a supported producer when a single ordinary pipe ends in numeric head or tail
• keep file-reading, follow-mode, multi-stage, and |& pipelines unchanged
• cover the Claude hook response and pipeline safety boundaries
