Add `patchwise --mail`, which watches a mailbox, reviews patches sent to a mailing list, and replies with the results. The mailer lives in the new patchwise/mail_handler package and is configured through a `mail:` block in the user config. The `--model`/`--provider` defaults now come from an `ai:` config block; both flags can still be passed on the command line to override it.
