fix(server): identify agent thread sends in provider prompts - #24759
Agent-authored thread sends already store createdBy and senderThreadId, but new provider turns and steers receive only the body. A message from another thread can therefore look like the user's instruction.
Change
Add one sender line when those two paths assemble ordinary agent messages for the provider. Use the sender's title or fall back to its thread id, and identify app-owned delegated children from the receiver's existing records. User messages and stored bodies remain unchanged. Flatten sender titles and neutralize currency sigils so metadata cannot invoke a skill, including Pi's numeric skill names.
To preserve native invocation without catalog lookups, text starting with a slash or a shared-grammar skill token after leading whitespace stays raw, so unknown commands, disabled skills, command-like prose such as $HOME, and file-bearing slash fallbacks also omit the sender line.
Scope and approval
Fixes#24711 (open, not yet triaged). This is a focused fix for provenance already recorded on thread messages: it carries existing metadata into the provider prompt and adds no workflow, setting, protocol or storage change.
This replaces #24483 with a separately written implementation.
Verification
Verified at 1051656c694d0c0a34041108483353c64e389074, based on upstream c79ef8c454e1268ff79da5a9dcb74572e86f7861:
Four focused orchestration suites: 105 passing tests.
Pi extension suite: 28 passing tests; the real input hook ignores deploy, 25, 123 and deploy.v2 in sender titles while body requests still load the skills. Restoring the previous sanitizer makes three regressions fail. Independent probes cover all 64 currency sigils and mixed/adjacent sigils.
Both delegated-task Codex replays pass, retain upstream's collaborationMode, and assert the original stored bodies and sender metadata.
Native-command probes pass at the Claude, Codex, Cursor, OpenCode, Pi, ACP and Grok adapter boundaries. Server tsc --noEmit, touched-file lint and vp run check:changed pass; the changed-file gate runs 418 passing tests. No live paid-provider session or UI check was run.
Agent-authored thread sends reached the provider as raw user text. Prefix one provenance line where new turns and steers assemble provider text, and leave the stored message unchanged.

…nce header
The header was the provider text, so /compact, Codex /goal, and a leading Grok skill no longer matched the handlers that already recognise them. Skip the header for those invocations, and expect it on the delegated-task replay.

Discovered slash commands and enabled skills stay raw only for the driver that would invoke them. Ordinary lookalikes still get a sender line, and a skill token in the title no longer dispatches.

Catalog lookups kept missing a driver's commands and delayed ordinary turns. Any agent message that starts with native syntax is sent unchanged, and every other agent message gets the sender line.

Pi loads any whitespace-delimited dollar token as a skill, including numeric names. A word joiner after each currency sigil keeps the title from matching while the body still can.

Macroscope's review found this PR approvable — Adds a centralized provenance header for existing agent-authored cross-thread messages while preserving user messages, stored bodies, and native command syntax. Production changes are limited to normal turns and steering; the remaining changes are tests, fixtures, or package export metadata.

Walkthrough
The change adds a shared formatter for eligible agent-authored provider text. New-turn and steering paths use it, while tests check sender attribution, delegated-task labels, native command and skill handling, and replayed message provenance.

Summary
Format sender provenance packages/shared/src/agentSendProvenance.ts, packages/shared/package.json, apps/server/src/orchestration-v2/agentSendProvenance.test.ts
Adds providerTextForTurn to format eligible agent-authored text with sender details and delegated-task labels. It preserves non-agent messages, messages without a sender thread, and text that starts with recognized native syntax. Tests cover title formatting, lookup fallbacks, and skill parsing.
Apply provenance to new turns apps/server/src/orchestration-v2/ProviderTurnStartService.ts, apps/server/src/orchestration-v2/ProviderTurnStartService.test.ts, apps/server/src/orchestration-v2/RunExecutionService.test.ts
New turns use formatted text and provide message provenance and sender and subagent lookup callbacks. Tests cover attribution, lookup failures, command handling, and compact routing.
Apply provenance to steering apps/server/src/orchestration-v2/ProviderTurnControlService.ts, apps/server/src/orchestration-v2/ProviderTurnControlService.test.ts
Steering uses formatted text with sender-title and receiver-subagent callbacks. Tests cover attribution, delegated-task labels, lookup failures, and pass-through cases.
Provider tests check agent-authored command and skill handling, including slash commands and skill references. Package exports expose the Grok skills and Pi commands modules.
The replay fixture includes sender attribution in delegated prompts. Integration tests check message authorship and sender thread, and confirm the final projection omits the attribution prefix.

Explanation
The title "fix(server): identify agent thread sends in provider prompts" matches the main change. The changeset adds a sender line to agent-authored messages in provider turns and steers, and the title names that behavior. It also follows the conventional commit format the template asks for. The title does not mention the skill-token and slash-command exemptions, but it does not need to cover every detail.
Full details: Description check
Explanation
The description follows the template and covers its main sections. Problem explains that agent-thread sends store createdBy and senderThreadId, but provider turns and steers receive only the body. Change describes the sender line, title fallback to thread id, app-owned delegated child detection, and the slash and skill-token exemptions. Scope and approval links issue #24711 and states the change is focused. Verification lists specific suites, test counts, and the commit at which they ran, and states that no live paid-provider session or UI check was run. The description does not include a template comment-free structure issue, and the verification section gives observed results rather than only saying tests pass. Minor gap: the Scope section cites an open, untriaged issue, while the template asks for triaged issue or maintainer approval. The author argues it qualifies as a focused fix of an obvious bug, but that argument is thin, so a maintainer should confirm approval.
Full details: Linked Issues check
Explanation
Issue [#24711] requires provider-bound agent messages to identify the sender and include the sender title or thread ID. providerTextForTurn adds an agent-authored header for messages with createdBy: "agent" and senderThreadId. It uses the sender title, falls back to the thread ID, and marks matching app_owned receiver children as delegated tasks. ProviderTurnStartService and ProviderTurnControlService apply the helper to new turns and steers. Tests cover sender lookup, fallback, delegated children, native command preservation, and unchanged user messages and stored bodies.
Full details: Out of Scope Changes check
Explanation
The changes remain within issue [#24711]. The shared helper and the two orchestration call sites carry existing sender provenance into provider text. Tests and replay fixtures verify the behavior. Package export changes support cross-package regression tests for native command handling. No unrelated workflow, setting, protocol, storage, or UI change is shown.

Move these tests into apps/server, or move the pure providerTextForTurn helper into @t3tools/shared. Moving the helper is likely narrower than relocating the provider-specific suites because all affected packages already depend on @t3tools/shared. This is a recommended boundary refactor even though no runtime test or typecheck failure is established.

Move the existing formatter into shared so provider tests use a package export instead of importing server application code. Keep provider coverage and prompt behavior unchanged.
