docs(pipeline): document iron-proxy MITM TLS inspection and CA trust

The Iron-Proxy stage description was sparse and didn't explain the MITM
mechanism or the client-side CA trust requirement — which is the #1
gotcha when wiring a new client through iron-proxy.

Added:
- Why MITM is needed (allowlist + audit happen at HTTP layer; without
  MITM, HTTPS is opaque and only SNI filtering is possible)
- How MITM works (on-the-fly leaf certs signed by iron-proxy's CA)
- Client CA trust requirement (mount ca.crt + NODE_EXTRA_CA_CERTS for
  Node.js clients like pi/pi-windsurf)
- Tunnel listener (port 8870) for CONNECT-based HTTPS proxying
- Upstream proxy chaining to NordVPN
- Verified chain: Pi → iron-proxy (MITM+audit) → NordVPN → Cognition
