Stacked on #27. Adds one extension point instead of built-in channels: LEAN_ATTENTION_ESCALATE_COMMAND runs your own command if nobody reacts within LEAN_ATTENTION_ESCALATE_AFTER seconds (default 120). UserPromptSubmit, PostToolUse, Stop and SessionEnd hooks cancel it; the cancel path returns in ~5 ms when nothing is pending, since it runs on every tool call. The README shows a KDE Connect phone ping as the example (the use case from #ai-workplace).

Tested on Linux: fire without reaction, cancel, cancel from another session, immediate (0), and hook registration via --plugin-dir (5 hooks).
