[guide] AGENTS.md still documents the per-file harness region floor as 93% — actual gate is 97% since #1250 #1306

Documentation Gap

AGENTS.md line 54 describes the tests/tools/ coverage harness floors as "(100% lines / 97% regions, 93% regions per harness file)". The per-file harness region floor was ratcheted from 93 to 97 by #1250 (merged 2026-10-10T20:01Z, commit 5c387fb), which changed package.json (--check-harness-file-regions 93 → --check-harness-file-regions 97) and tests/coverage-gate-thresholds.test.mjs but did not update AGENTS.md.

Verified at main 4184f33: package.json's test:unit:coverage:check passes --check-harness-file-regions 97, while AGENTS.md:54 still says 93.

Impact

Agents and contributors read AGENTS.md as the entry point for the coverage contract. A stale floor number means someone calibrating a change against the documented 93% per-file floor can be surprised by CI failing at 97% — the doc understates the gate by 4 points.

Recommendation

Update AGENTS.md:54 to say 97% regions per harness file, matching the current --check-harness-file-regions 97 flag in package.json. Single-line change.
