feat: replay completed-turn tool history in context assembly - #10

Conventional agents keep a single flat append-only transcript where tool_call and tool_result are first-class, permanent entries. santi dropped them from assembly_input, so across turns the soul lost all record of tools it ran (a real gap on the live chat_completions path, not just a Responses-acceptable simplification). ProviderMessage becomes an enum (Text / ToolCalls / ToolResult). assembly_input now replays completed-turn tool_call/tool_result inline, bounded by tools_through_seq (the in-flight turn's base_soul_session_seq): in-flight calls still flow through function_call_outputs untouched, so GLM-5.2's reasoning_content echo and the OpenAI Responses raw item are preserved with zero behavior change on the live in-flight path. thinking stays dropped cross-turn.
