Move the ad out of the footer to the top of the content (#69)

Anthony: no ads in footers. The root layout rendered four data-cp-ad units (300x250, 728x90, 320x50, text_link) after <Footer/>. Replaced them with one unit at the top of <main> (no data-format, so ad.js sizes it to the container). ad.js loader unchanged.
