Add activity to personal profile

• Create component 'add activity button'
• Render 'add event button' (fixed position on mobile and desktop)
• When button clicked - display event input form
• Display event input form 2 inptus (name of event and date completed)
• Display event input form next 2 inptus (activity type and duration)
• Display event input form next 2 inptus (related skills and confirmation link)
• On submit hide form and submit date to airtable
• Re-render activity log with latest data pushed to top
